Media Release form. This form allows St. Thomas to post photographs of your child in publications, multimedia productions, displays, advertisements or internet publication related to St. Thomas.
Student Medical Form. This forms will notify St. Thomas of your child's medical history as needed for various activities at St. Thomas or traveled to events.
Student Permission Form. This form describes the expected student rules of conduct, permission to seek medical attention if necessary and the consent for your child to attend an activity.
